This is an example of a Stage 4 pressure injury located on a sacrum on a mannequin. This represents full thickness tissue loss with extensive tissue destruction, tissue necrosis or damage to muscle, bone or supporting structures such as the tendon or joint capsule. So as you can see in this mannequin here we have bone on view. We have full thickness skin loss, the wound depth being about 1cm in depth and there’s undermining or tunnelling of the surrounding skin.
